Rehabilitation therapy team:

Physical Therapy May Include:
• Continuation of post-operative physical therapy
• Treatment of orthopedic injuries
• Therapeutic exercise for strengthening
• Bed mobility training
• Transfer training
• Ambulation training
• Teaching the use of walker, cane, crutches
• Durable Medical Equipment (DME) coordination
• Fall prevention training
• Pulmonary/Cardiac rehabilitation

Occupational Therapy May Include:
• Therapeutic exercise related to upper extremity
activities and function
• Measures to improve self-care, dressing, and
activities of daily living
• Teaching of energy conservation measures to
improve endurance
• Assessment and teaching the use of splints and other DME
• Arrangement of environment to improve home safety
and increase independence

Speech Therapy May Include Treatment Of:
• Speech disorder
• Swallowing and/or choking problems
• Esophageal disorder
• Memory deficits
• Cognitive re-learning
...and may include the use of the Lee Silverman voice
treatment (LSVT)

Social Worker Services May Include:
• Emotional support to cope with problems relating to
the patient’s illness
• Information regarding community resources, financial
assistance, fiduciary services and power of attorney
• Assistance with financial consultation for financial aid
• Assistance with identifying long term care needs and
placement if needed

Skilled Nurses Services Team:
• Registered nurses
• Licensed practical nurses
• Home health aides

Nursing Services May Include:
• Comprehensive nursing assessment and development
of a plan of care in coordination with the physician
• Specialized patient and caregiver teaching
• Medication teaching to include administration and
teaching injectable medications
• Urinary catheter care
• Diabetic teaching and management
• Colostomy teaching and management
• Pain control
• Wound management (nurses are certified in VAC
for wound care)

Home Health Aid Services May Include:
• Bathing: bed, bath or shower
• Skin and nail care
• Dressing
• Range of motion exercises
• Assistance with transfer and ambulation
All under the supervision of a registered nurse and/or
physical therapist.
